Read on as Dr. Tif Qureshi, Prav Solanki, and Dr. Payman Langroudi discuss.

Dr. Payman Langroudi: Where do you get the confidence to go to the best orthodontist in the world as a GDP and say: "I'll do this. What do you think?".

Dr. Tif Qureshi: Here you have to thank people like BACD. You too have this opportunity and you should always be grateful for it in reality.

But the reality is that I was thinking that way, and I was thinking that way in an environment where other people weren't. And I'm not saying it would have been exactly the same, but I was heading in that direction anyway. So I would have talked to one person or another. I just wanted to validate it and make sure that it was something I was doing as well as I could.

Dr. Payman Langroudi: Have you ever thought about it, or maybe I've even heard you or read about you saying this. But today it would be impossible, because you would be so afraid of the legal ramifications of being the only one doing something.

“A general dentist who doesn’t do orthopedics is dangerous.”

Dr. Tif Qureshi: I think you're absolutely right, Pay. And I think that's actually a pretty sad statement actually. We've all been lucky to a certain extent for the times we've lived in. But these days, yeah, you'd probably be afraid.

However, I've come to the point where I'm just turning this argument around, where I used to hear and still hear people say, "If a general dentist does orthopedics, it's dangerous.". Well, actually, now I'm turning the argument around and saying, "A general dentist who doesn't do orthopedics is dangerous, okay?"“.

And I can justify this on the basis that many dentists don't understand the concept of constrictive envelopes. And why do the envelopes constrict? Because the teeth move.

And how do you fix this? Not through restoration. Yes, you can do restoration work, but you need orthopedics.

And there are probably, I would estimate, millions of patients around the world who have a front tooth that repeatedly chipped. And the dentist's solutions are another composite, another composite, or a splint, and many of those patients end up with crowns.

And if the dentist understood that they needed a little bit of orthodontics to get the overpower right, the right overpower, a little bit of bonding to improve the overpower, you would probably find that in fact a lot fewer people would have anterior veneers or crowns. I saw that. It's absolutely clear, and I think it's a very important part of the message right now.

So what I would say, and this is controversial, but if you're a general dentist, and you're my general dentist, and you couldn't do orthopedics, I wouldn't even sit in your chair.

That's what I would say. I wouldn't even sit in your chair. If you needed to do endodontics, I would be okay with that, but if it was a front tooth, and your solution for the front tooth that keeps chipping was a crown, there's no way I would sit in your chair again.

Dr. Payman Langroudi: The thing is, saying it now, with your record, you can basically say whatever you want and everyone will pay attention. But back then, saying stuff like that like Kent GDP, I find it super interesting.

Dr. Tif Qureshi: Well, we were saying things. And we were actually putting ourselves out there and showing what we were doing. And you know if you stick your head out there, there are going to be people who want to take you down.

I got a few invitations to places where I was terrified the audience would be full of orthodontists. But the reality was, when they sat and watched what I was doing, a lot of them came up to me afterwards and said, "Actually, "You're just doing the front teeth. You're looking at what's actually happening."”. So the key part here, and this is where people like Ross Hobson have made a huge difference, is actually to validate that what we're trying to do is make sure that patients understand the difference between a complete treatment and a compromise. That's the key.

As long as you can fully diagnose the case, and you have to diagnose it yourself, not someone else. This is very important and you can explain to the patient why it is good to do just the front teeth, compared to what the patient might really need, which is to do a full mouth. It really shouldn't be a problem at all. And once I brought that message out, I think it became clear that there's nothing wrong with doing it. And that you should actually start thinking about doing it if you're not already doing it.

Prav Solanki: So, let's say you found this device, you got it working, you started changing teeth, doing fewer veneers. And fast forward, you put together an academy.

Dr. Payman Langroudi: Direct discussion seminars, I seem to remember.

Dr. Tif Qureshi: That's how it was. And this is a good time to bring in James Russell and Tim Bradstock-Smith, because they were two of the first people who actually went to the first "study club," where it was just them and me. We had our own little study club, and within a week we were talking about a different topic. I think when we got to the fifth topic, which was just the Inman Aligner, so I did composite, and Tim did occlusion, and James did temporary veneers. We were lecturing each other. It's a little sad when you think about it.

But When Inman Aligner appeared, they were left speechless, like: “What the hell is this? Wow.” And all of a sudden, they wanted to start learning how to use it. I kind of trained them a little bit on it. They didn't need much. And then before we knew it, we were running a course as a kind of experiment for some of the people and some of the BACD people, and it just took off from there. But it was very rough and very innocent. We were trying to do the right thing, but equally, as things have changed now, we've developed it to do everything the right way.

Probably the best thing about it was that you couldn't do much with it. It wasn't a treatment that could treat a lot of teeth. You could see right away, if the back teeth needed treatment, it's not the right treatment for that. Simple as that.

“It’s the old cliché. If you do the right thing, the money will follow you. And it is.”

Dr. Payman Langroudi: Do you remember the moment when you thought that instead of being a treatment modality, it could be a business?

Dr. Tif Qureshi: I think there was never an artificial way to do this. And that's probably a weakness in itself, in the sense that we could have been much more advanced than we are. But the truth is, a good few years ago, maybe five or six years ago, I think once we started doing courses... Actually, it's more than that. We've been doing courses in Denmark for about eight or nine years. But once we started going abroad, and of course a lot of it was just because you get an invitation, because it's a bit of fun and things like that.

But Once we started going abroad and we started getting a lot of feedback from dentists saying, “This has completely changed the way I do dentistry,” I think we realized we had a kind of responsibility to do something and set up something more formal. So we set up the support forum and all of that stuff and it grew from there.

It's a business now, and we have to make sure it's running and operating properly. But it's still largely run by dentists, so we have our ethical hats on all the time. Sometimes we probably do things that a tough business would tell us, “Don’t do this, do this instead.” But we can’t. We just have to do things that might hold us back in the long run. But ultimately, I have to stand up and look in the mirror, and I know that the other guys have to do the same. So in a way, that's why I took it the way I did.

Prav Solanki: So, ethics over profits.

Dr. Tif Qureshi: I think that's always been a good message, not just in what we do, but in dentistry in general. A lot of people believe that and they want to believe it and they want to say it louder. It's the old cliché, if you do the right thing, the money will follow you. And so it is.
