Dr. Linda Greenwall discusses the strict guidelines for teeth whitening, and Dr. Rahul Nehra tells us how he managed to circumvent the legalities to transform an 11-year-old's life by whitening a single tooth.<\/strong><\/p>\n\n\n\n

Teeth whitening, despite its popularity, is heavily regulated in the UK and is an illegal procedure for those under 18, but what does it mean for children with white spots on their teeth or a single dark tooth who would be bullied if they didn't get their teeth whitened?<\/p>\n\n\n\n

Dr. Linda Greenwall talks to us about the main guidelines and how they are working to change them.<\/strong><\/p>\n\n\n\n

There are several issues that revolve around teeth whitening under the age of 18. These days there is a 20 to 40 percent increase in children having white spots on their teeth. This means that we, as dentists, are faced with the challenge of coming up with strategies to overcome this. As with any dental procedure, we must remember that we must have the best interest of the patients in mind. More importantly, however, we must keep in mind that for children who have white spots on their teeth faded<\/strong>, this has an impact on their daily lives and we must do everything we can to provide them with a solution. As a result of discolouration<\/strong>, many of them often have low self-esteem issues and may face some form of bullying at school.<\/p>\n\n\n\n

Guidelines on how under-18s can be treated with teeth whitening are set by the General Dental Council. While European bodies prohibit any child under 18 from receiving treatment under any circumstances; laws need to be changed to provide solutions in exceptional cases. However, the General Dental Council's guidance states that if there is a specific condition that a child is suffering from, then treatment is permitted.<\/p>\n\n\n\n

There are 8 to 10 categories where whitening is appropriate if a child is suffering from a disease. One case where whitening would be necessary is in the case of trauma, when a child has suffered a wound and the tooth has turned black. This would require the patient to undergo root canal treatment and, due to the black tooth, it would need to be internally bleached. Other common causes would be white spots, brown spots, amelogenesis imperfecta and fluorosis. What is our new strategy?<\/strong><\/p>\n\n\n\n

At the British Dental Bleaching Society we aim to reform the legislation. One way to do this would be to rewriting the document that was presented for scientific evidence.<\/strong> It is on our agenda that it be amended and re-introduced to re-ensure its security, which can then be channelled through Europe. Another way we are looking to address this situation, post-Brexit, is let's make a petition<\/strong> in an attempt to encourage change. We must remember that a child's right to health is a fundamental human right. This means that treatment cannot be refused if a condition is detrimental to their health and is a key reason why we hope to influence legislation.<\/p>\n\n\n\n

We are so convinced of this issue that we are organising a conference on 15th November at Lords Cricket ground. Key researcher Kevin Donlley, from Texas, who has undertaken extensive research into bleaching under 18s and its safety, will be presenting his evidence. We will detail the 8 different categories where bleaching is appropriate, as well as case studies of the types of treatment that can be carried out in a minimally invasive way.<\/p>\n\n\n\n

The difficult case of a young patient<\/strong><\/p>\n\n\n\n

Only a GDC-registered dentist can perform teeth whitening on patients. But it is still illegal for anyone under the age of 18. So when Dr Rahul Nehra found himself with an interesting case of a child being assaulted on the head, he had to seek advice from several bodies before he could offer his young patient the most conservative treatment for his particular case.<\/p>\n\n\n\n

Read on as Dr. Rahul Nehra sits down with Dr. Payman Langroudi to tell the whole story.<\/p>\n\n\n\n

Payman:<\/strong> Rahul recently found himself in an interesting situation where he had a child patient who needed teeth whitening and because of that he found himself on the wrong side of the law.<\/p>\n\n\n\n

Maybe you can give us more details, Rahul, tell us the whole story.<\/p>\n\n\n\n

Rahul:<\/strong> So, about 18 months ago, an 11-year-old fell at school, as most children do, but unfortunately, the fall knocked out his upper left tooth<\/strong>. It turned completely black.<\/p>\n\n\n\n

We asked the endodontist to perform a root canal treatment, but unfortunately the child was being bullied at school and it was very difficult for him to go to school afterwards. His parents started looking at quite extreme options, such as would be the facets<\/strong> and homeschooling.<\/strong><\/p>\n\n\n\n

How teeth whitening became an option<\/strong><\/p>\n\n\n\n

Then we intervened and told them that there was simpler options<\/strong> and that we can simply whiten his teeth inside and out<\/strong>. I knew that using Enlighten would work, so I contacted you and got a response within a few hours. The parents agreed and I spoke to the Dental Defense Union, who agreed to go ahead because we were doing something to protect the child and we had the parents' consent.<\/p>\n\n\n\n

The results were incredible.<\/strong><\/p>\n\n\n\n

Payman:<\/strong> Did your parents initially come to you asking for veneers?<\/p>\n\n\n\n

Rahul:<\/strong> Yes. I had several consultations where they said they wanted veneers or teeth straightening, anything that could just improve the way the tooth looked. They made it clear to me that if I didn't want to help them, then they would find another dentist. So I had to explain to them that it wasn't just up to me and that the decision came from somewhere else.<\/p>\n\n\n\n

After talking to a few different companies, you and then the defense union, we were told to go ahead because the results would show themselves. The parents were very pleased with the results, and the child has integrated back into school.<\/strong><\/p>\n\n\n\n

Payman:<\/strong> So, you are in a difficult situation, where it is legal to prepare this 11-year-old's teeth<\/strong>, but it is illegal to do something as conservative as bleaching<\/strong>.<\/p>\n\n\n\n

Rahul:<\/strong> Exact.<\/p>\n\n\n\n

Payman:<\/strong> And how did you feel?<\/p>\n\n\n\n

Rahul:<\/strong> Strange, very strange. It's a cavity with an axis, it's a small axis. And the thing is, the axial cavity was already produced by the endodontist. I didn't even cut the tooth. And the mother was happy to do the whitening. Both sets of parents had individual meetings with me to tell me that you have to protect my son, that you have to help him.<\/p>\n\n\n\n

Payman:<\/strong> How serious was it?<\/p>\n\n\n\n

Rahul:<\/strong> It was bad. He would come and even when he talked to us as adults he would slowly calm down. He wasn't the same as before.<\/p>\n\n\n\n

Teeth Whitening Law<\/strong><\/p>\n\n\n\n

Payman:<\/strong> I think GDC has now said that if fading<\/strong> it's due to a disease process, then you can do what you did. So, I think ultimately it would have been okay, but the situation definitely needed to be clarified.<\/p>\n\n\n\n

\u201cSomething so basic and simple improved his lifestyle.\u201d<\/strong><\/p>\n\n\n\n

And it reminds me of 2001 to 2012, when bleaching was illegal<\/strong> in the UK. A few of us were providing teeth whitening products to patients and we got together: Linda Greenwell, Chris Law, Martin Canada and the idea was that if it went to court we would be willing to support it because ultimately your main concern is the well-being of your patients. So I can understand why you went ahead. But the situation needs to be clarified and Linda is doing something about that now by finding twelve situations where it is okay to whiten the teeth of someone under 18.<\/p>\n\n\n\n

Rahul:<\/strong> Something needs to be done. It was such a basic treatment and everyone is happy. The parents are delighted, the child is delighted. He didn't need to whiten the rest of his teeth, he literally had one tooth whitened and that was it. something so simple and basic improved his lifestyle<\/strong>.<\/p>\n\n\n\n

Payman:<\/strong> I mean, you're trying to do everything you can for this patient in this difficult situation.<\/p>\n\n\n\n

Rahul:<\/strong> If it were my 11-year-old, I wouldn't have prepared a crown for him. So why would I prepare a crown for someone else's 11-year-old? If it's as simple as putting in a filling and a little whitening. You can't go wrong.<\/p>\n\n\n\n

Payman: <\/strong>What steps did you take before moving forward?<\/p>\n\n\n\n

Rahul:<\/strong> I gave my patient all the options. Both parents were involved in the process. I sent them home with the whitening, the veneer, and the option to see a specialist. They decided that whitening would be the option they wanted to choose. They realized it was the most conservative <\/strong>and that rthe results will be the best.<\/strong> We gave them a grace period and sent them the documentation. We then spoke to the defense union, who were happy to move forward.<\/p>\n\n\n\n

Payman<\/strong>: What defense union was it about?<\/p>\n\n\n\n

Rahul:<\/strong> TDS defense union and they were very good. They said as long as you have done all the correct steps. Go ahead.<\/p>\n\n\n\n

After that, what we did was send the patient away and let him make his own decision, and once the decision was made, I went ahead and did the treatment, because my patient's best interest is all I care about.<\/p>\n\n\n\n

The result<\/strong><\/p>\n\n\n\n

Payman: <\/strong>And you have reached the result you wanted.<\/p>\n\n\n\n

Rahul:<\/strong> We achieved the result, and the patient is overjoyed.<\/p>\n\n\n\n

Payman: <\/strong>Well, let's hope this will be one of the cases that Linda can use in her campaign, she's trying to get this law changed<\/strong>, so thatWe can treat people younger than 18 in the right situations.<\/strong>, because we know it's completely safe. Whereas the other option would have been completely unsafe.<\/p>\n\n\n\n

Rahul:<\/strong> My team went through this too. Everyone at the smile gallery said this was the best option. I showed them before and after pictures of the previous whitening I had done. And if it works on an adult, why wouldn't it work on a child?<\/p>\n\n\n\n
